Pain medicine
To relieve pain in CFS patients is one of the priorities. The most frequent used medications include naproxen (Aleve, Anaprox, Naprosen), ibuprofen (Advil, Bayer Select, Motrin, Nuprin), and piroxicam (Feldene). Are also prescribed drugs like: tramadol hydrochloride (Ultram), celecoxib (Celebrex), and refecoxib (Vioxx), generally safe when used as directed, but with a variety of adverse effects, including kidney damage, gastrointestinal bleeding, abdominal pain, nausea, and vomiting.
Non-steroidal anti-inflammatory drugs (NSAIDs) as ibuprofen represent the standard pain relievers, but the problem is that it can cause upset stomach and even ulcers. Sometimes other NSAIDs work better than ibuprofen because can be taken regularly and less frequently during the day. Diclofenac (Voltaren), the long-acting Voltaren XL or Sulindac (Clinoril) are relevant examples.
Tricyclics (doxepin) are drugs used as antidepressants and for improving pain, but must be taken regularly, good days and bad, to be effective. CFS patients are usually sensitive to them, and lower starting doses should be used.
Tramadol (Ultram) should be used with caution in conjunction with Prozac and tricyclics because has some effect on the serotonin and norepinephrine systems. But is unlikely to cause dependence and it is worth a try. Baclofen is a nifty drug – it’s cheap, but often it doesn’t work. It is best when the muscle pain is of a cramping or spasm quality.
Neurontin (Gabapentin) is a new medication prescribed for CFS patients, but it should be reviewed carefully before use because it may cause dizziness or increase fatigue
A good pain relief is sometimes Carbamazepine (Tegretol or Tegretol XR). But this kind of medication can cause excitation, bone marrow problems and allergic reactions and should not be used with erythromycin, Prozac and other drugs.
Narcotics are used to treat severe pain, and intermittent use is recommended because reduces the risk of addiction. Propoxyphene (Darvon) should be avoided, because it may have a high addiction potential and offers wimpy pain relief. Ultram is preferred because of its low addiction potential and good efficacy.
A "medicine" with great effect in reducing pain is increasing water intake (along with salt). It is an inexpensive, safe and easy method.
